What is botulinum toxin and how does it work?

Botulinum toxin type A

— commercially known as 

Botox®, Azzalure® or Bocouture® - is a purified neurotoxin derived from Clostridium botulinum.

 

In controlled medical doses, it temporarily blocks the release of acetylcholine at the neuromuscular junction — the point where a nerve communicates with a muscle — preventing contraction of the targeted muscle (Halk et al., 2021).

To understand it, imagine what happens every time you want to contract a muscle: your brain sends an electrical impulse through a nerve to that muscle. When the impulse reaches the 'neuromuscular junction', the electric signal converts from electrical to chemical, allowing the muscle to contract.

When Botulinum toxin is injected, it temporarily blocks this conversion, preventing the release of that chemical messenger — acetylcholine.

As a result, the muscle doesn’t receive the message to contract.

This process is entirely reversible. Over time, the body recognises that communication has paused for whatever the reason and forms new nerve endings, restoring normal function through a process known as synaptic sprouting.

Think of it like unplugging a landline phone — the phone isn’t damaged; it’s simply disconnected for a while. Once reconnected, communication resumes as normal. That’s precisely how Botulinum toxin works: controlled, reversible, and entirely physiological.

Clinically, this translates into the softening of dynamic wrinkles — lines that appear when muscles are actively engaged, such as during frowning, squinting, or raising the eyebrows.

With repeated movement, these can eventually develop into static wrinkles, which remain visible even at rest.

Botulinum toxin effectively addresses the muscle-driven component of these lines, not those caused by volume loss, skin laxity, or collagen depletion, which require different treatments.

The goal isn’t paralysis — it’s refinement. Achieving natural, balanced results depends on anatomical precision: knowing which muscles to treat, where they originate and insert, and how they influence expression — combined with an in-depth understanding of the product and spatial awareness during injection.

My fascination with botulinum toxin and its mechanism began years ago, when I first wrote a final-year project on the medical use of Botulinum toxin at the end of college. Later, during my master’s degree in Cosmetic and Aesthetic Medicine, I compared the clinical performance of different Botulinum toxin brands in my thesis. This foundation, combined with my background as a Master of Pharmacy, gives me a unique perspective — one that merges pharmacological understanding with anatomical precision.

The Treatment experience

If you arrive on time for your appointment (please, do so) there’s always enough time to settle in, ask any remaining questions, and review documentation before we begin.

 

All consent and medical forms are sent to you in advance, so you can familiarise yourself with them beforehand — but if anything needs clarification, I’ll take the time to explain it in full.

 

topical anaesthetic cream is then applied for comfort. This step takes around 10 - 15 minutes and, although many practitioners now skip it to save time, I choose to include it.

Your comfort matters (!) — and in my practice, it’s part of the treatment. I take time to ensure you’re relaxed and confident before we begin.

 

Once the skin is comfortably numb, the treatment begins. Using ultra-fine 34G sterile needles, appropriate amounts of the medicine are placed precisely into the targeted facial muscles.

The injection process itself takes around 15 - 20 minutes, depending on the treatment area. In total, your appointment will last approximately 45 - 60 minutes, allowing enough time for conversation, assessment and treatment.

 

Throughout the procedure, I record detailed notes — including the exact product, quantity, and injection sites — ensuring that every future treatment builds on accurate data.

 

You can resume normal activity straight away, though vigorous exercise, alcohol, or facial massage should be avoided for 24 hours (British Association of Dermatologists, 2022).

Clinical Standards

Botulinum toxin is a prescription-only medicine (POM), administered only after a face-to-face consultation with a qualified prescriber. 

At odNOVA Aesthetics, treatments are delivered by an Independent Prescribing Pharmacist with MSc in Cosmetic and Aesthetic Medicine and ongoing study in Dermatology.

All procedures follow Joint Council for Cosmetic Practitioners (JCCP) standards.

Aftercare and Maintenance.

  • Avoid heat exposure, vigorous exercise, and facial massage for 24 hours.

  • Stay upright for at least 4 hours post-treatment.

  • Skip alcohol and blood-thinning medication on the day if possible.

  • Refrain from applying makeup until the next day.

  • Use daily SPF 50 to protect results (British Journal of Dermatology, 2020).

A review at two weeks ensures even distribution and natural expression.

Long-term maintenance is usually every 3 - 4 months, often complemented by treatments such as skin boosters or gentle chemical peels to improve skin texture and longevity.

Why Might Botox Seem Less Effective?

One of the most common reasons for Botox appearing less effective is dosage. It’s not a one-size-fits-all treatment — the number of units required varies with the area treated, muscle strength, and your desired outcome. Stronger or more active muscles, such as the glabellar complex or masseters, often need higher doses to achieve optimal relaxation.

Beyond dosage, several other factors can influence results:

  • Injection placement and precision — even minor deviations in depth or positioning can alter effectiveness.

  • Individual response — metabolic rate, muscle anatomy, and previous exposure to botulinum toxin can affect how long results last.

Resistance development — although uncommon, some individuals may gradually develop antibodies that reduce the toxin’s effect.​

Is Botox dangerous ? 

When administered correctly by an appropriately qualified healthcare professional, Botox is considered a very safe medical treatment with a long and well-established safety record.

Botulinum toxin has been used in medicine for over 30 years to treat neurological, ophthalmic and pain-related conditions, often in significantly higher doses than those used for cosmetic treatments.

Possible short-term side effects

Most clients experience no downtime, but temporary effects can include:

  • mild redness or swelling at the injection site

  • slight bruising

  • transient headache

  • a feeling of heaviness in the treated area

These effects are usually short-lived and settle without intervention.

When can Botox become unsafe?

Botox can carry risk only when:

  • the consultation is not performed by a qualified prescriber

  • incorrect products or unlicensed sources (!) are used

  • treatment is carried out by someone without medical training in facial anatomy

  • sterile technique and clinical protocols are not followed

This is why choosing a medically trained, prescribing clinician is essential for safe, predictable outcomes.

Who can administer Botox ? 

Botulinum toxin (aka. Botox) is a prescription-only medicine in the UK, which means it must first be prescribed following a face-to-face consultation by an appropriately qualified healthcare professional with prescribing rights — such as a medical doctor, pharmacist independent prescriber, or nurse independent prescriber (NICE, 2024).
Although (sadly) UK law does not currently restrict who may physically inject botulinum toxin, the practitioner administering it must do so under the oversight and legal responsibility of the prescriber (NHS, 2023).
In practice, this means that while almost anyone can attend a short “Botox course”, they cannot prescribe the medicine and may only inject under a prescriber’s direction and within a clinical governance framework.

How this works at odNOVA Aesthetics
At odNOVA, you will always receive a face-to-face consultation with me, followed by treatment performed by me — a Pharmacist Independent Prescriber with:

  • Master’s degree in Cosmetic & Aesthetic Medicine,

  • ongoing education in Clinical Dermatology,

  • clinician with advanced medical, anatomical and dermatology-led expertise.


Practitioners with this level of formal education are rare in the aesthetics field. Your entire treatment journey — consultation, prescribing and injecting — is overseen and delivered by the same highly trained clinician, ensuring safety, consistency and accountability.

Who should NOT use Botox ? 

While most people can safely receive treatment, there are certain situations where Botox may not be appropriate.

You may not be suitable for Botox if you:

  • are pregnant or breastfeeding 

  • have a known neuromuscular disorder, such as myasthenia gravis or Lambert–Eaton syndrome

  • have a history of severe allergy or hypersensitivity to botulinum toxin or any components of the product

  • currently have an active skin infection, rash, or inflammation at the planned injection site

  • have a significant bleeding disorder or are taking medications that severely increase bleeding risk

  • are experiencing unrealistic expectations or seeking treatment for reasons that wouldn’t benefit from botulinum toxin
     

Temporary reasons to delay treatment
Even if you are normally suitable for Botox, treatment may need to be postponed if you have:

  • a current cold, flu or fever

  • ongoing sinus infection or facial infection

  • recent dental work (particularly in the lower face/jawline treatments)

  • recent vaccinations or upcoming medical procedures

  • significant upcoming events where potential minor swelling or bruising would be an issue

Which Botox is the best ? 

Think of Botox brands like cars. Yes, cars!
While they all serve the same purpose — getting you from point A to point B (or in this case, smoothing out wrinkles) — each brand and model (toxin type) comes with its own features, performance profile and aesthetic finish.

All licensed botulinum toxin-A products used in the UK  — work in a similar way and consistently deliver effective results when used correctly.
But each has subtle differences in onset, spread, longevity and formulation, which means one may be better suited to your individual goals than another.

A fact about me
My Master’s thesis was actually focused on comparing different brands of botulinum toxin — so I’ve really dived deep into this area.
I understand the nuances between products in a way that goes far beyond brand marketing.

So which one is the best? There’s no one-size-fits-all answer.
The “best” toxin is the one that matches:

  • your anatomy (area being treated)

  • your previous response (if any)

  • your treatment goals

  • the dose and technique used

  • and the injector’s expertise
     

In other words: it’s not just about the car — it’s about the driver, the road, and where you want to go.

At odNOVA I use ONLY UK-licensed, pharmacy-sourced botulinum toxin brands, chosen specifically for what you want to achieve.
During your consultation, I will explain exactly why I recommend a particular product, and how it aligns with your anatomy and goals.

Which Botox lasts the longest ?

All licensed botulinum toxin type-A products used in the UK — including Botox®, Bocouture®, Dysport®, Azzalure®, and Xeomin® — generally provide results that last around 3- 4 months, although some people may enjoy results for up to 5- 6 months depending on their metabolism, dosage and muscle strength.

The honest answer
No brand is scientifically proven to consistently last longer than the others across all patients. Studies show that when equivalent doses are used, the longevity between products is very similar.

However, certain factors can influence perceived longevity:
1. Dysport® / Azzalure® — slightly quicker onset, sometimes “feels” longer in certain areas.
Some individuals report that Dysport/Azzalure wears off a little more slowly in larger muscles (forehead, masseter) due to its diffusion characteristics — but this is patient-dependent and not guaranteed.

2. Bocouture® / Xeomin® — minimal accessory proteins
These “purer” formulations may have a theoretical advantage in long-term use by reducing antibody formation, potentially supporting consistent longevity over many years. 
This doesn’t mean it lasts longer — just that it may keep working reliably even after many cycles.

3. Botox® — has the longest clinical history and vast research base. Its longevity is solid, predictable, and consistent.

Where can Botox be injected ? 

Botox isn’t just for frown lines — it is one of the most versatile prescription treatments in aesthetic and medical practice. It works by softening overactive muscles, improving symmetry, reducing wrinkles and refining facial balance.

How does Botox help migraines ? 

Botox is an approved medical treatment for chronic migraine — meaning 15 or more headache days per month — and has been shown to significantly reduce both the frequency and intensity of migraine episodes (NICE, 2021).

How it works - Botox helps migraines through several mechanisms:
1. It calms overactive pain pathways
Botulinum toxin blocks the release of neurotransmitters involved in pain signalling, such as CGRP and glutamate. This effectively “quiets down” the nerves responsible for triggering migraines (BNF, 2025).
2. It relaxes the muscles that contribute to tension
For some migraine sufferers, overactive muscles — especially around the forehead, temples, neck and shoulders — can act as triggers. Botox helps reduce this muscular tension.
3. It reduces peripheral nerve sensitivity
By dampening nerve activity in the areas where migraines begin, Botox reduces the likelihood that migraines will escalate into full attacks.

What areas are treated for migraines?
This differs from cosmetic treatment.
Medical migraine protocols usually include injections across forehead, glabella (frown area), temples, back of the head (occipital region), upper neck, trapezius muscles, sometimes the jaw muscles (masseter). This distributes the effect across the pain-trigger zones.

How quickly does it work?
Most clients begin to notice improvement after 2 - 4 weeks, with results building over 3 months. Treatment is repeated roughly every 12 weeks to maintain benefit.

In short:
Botox helps migraines by reducing nerve hyperactivity, calming pain signals, and relaxing overactive muscles. It’s a medically recognised treatment for chronic migraine and has helped many people reduce the frequency and severity of their symptoms.

When does Botox goes wrong ? 

Botox is a very safe treatment when performed by a qualified prescriber with medical training in anatomy, sterile technique and dosing (NICE, 2024). However, complications can occur — and almost always do so when treatment is delivered by poorly trained or unregulated injectors, or when inappropriate products are used (NHS, 2023; MHRA, 2022).

Botox tends to “go wrong” in situations such as:
1. Incorrect injectable technique: injecting too high/ too low/ too deep, failing to understand the balance between opposing muscle groups, using a fixed template rather than an individualised assessment

2. Wrong dose or poor product reconstitution
Over-dilution, under-dosing, or inconsistent mixing can lead to weak or short-lived results.
3. Using counterfeit, grey-market or unlicensed products
MHRA has issued multiple warnings about counterfeit botulinum toxin in the UK market (MHRA, 2022). These products are unsafe and may cause allergic reactions, contamination risks, unpredictable spread or absence of effect.
4. Injections performed by non-prescribers without medical oversight Complications are far more common when treatment is delivered by:

5. Treating unsuitable candidates
Botox can “go wrong” when administered to people with: 
untreated ptosis, pre-existing asymmetry, neuromuscular disorders, unrealistic expectations, extremely strong frontalis activation patterns, eyebrow descent with age (where aggressive forehead dosing worsens heaviness). !A proper medical consultation prevents this.
6. Poor aftercare or incorrect timing of follow-up
Massaging the area, strenuous exercise immediately after injections, or lying flat can cause unwanted diffusion. 

When does Botox wears off ?

Botox typically lasts 3 - 4 months, although some people notice results for up to 5 - 6 months, depending on their metabolism, muscle strength, lifestyle and the area treated.

You’ll usually feel movement gradually returning rather than a sudden “switching off.” The first areas to wear off are often those with strong or expressive muscles, such as the frown lines, while finer areas like crow’s feet may last slightly longer.

Does Botox last longer with regular treatments?
Yes — and here’s why:

With consistent treatments, the targeted muscles become weaker and smaller over time (muscle atrophy). Because they’re no longer constantly contracting at full force, the effects of Botox tend to last longer, and create smoother, more stable results (BNF, 2025).

What happens when it wears off?
Your muscles simply return to their natural activity. There is no sudden rebound, and your wrinkles do not worsen due to Botox — they simply return to how they looked before treatment (NHS, 2023).

How Botox helps migraines ?

For those of us who've gone through the wringer with migraines, finding a treatment that actually reduces the frequency can be life-changing. While it's not a cure-all, it's another tool in the arsenal against chronic migraines. Botox, or Botulinum toxin, works by blocking certain chemical signals from nerves, primarily those signals that cause muscle contraction. The theory for migraine relief is that Botox blocks the neurotransmitters that carry pain signals from your brain, effectively stopping the headache before it starts.

When Botox goes wrong ?

A lot of the horror stories we hear stem from two main issues: improper administration and unrealistic expectations. * Improper Administration: Not all practitioners have the same level of expertise. An inexperienced or unqualified provider might inject the wrong muscles, use incorrect dosages, or not consider a patient's unique facial anatomy. * Unrealistic Expectations: Botox isn't a one-size-fits-all solution. What works wonders for one person might not be suitable for another. Communication with your provider about what you hope to achieve is crucial.

When Botox wears off ?

The duration can vary from person to person, but on average, the effects last about 3 to 4 months. Here are a few signs that your Botox is wearing off: movements returns, fine lines reappear.

When Botox starts working ? 

From my experience and according to most publications, you can expect to see results within 3-7 days after the procedure. However, it can take up to two weeks to see the full effect. Patience is key here!

Can Botox cause cancer ?

According to the current body of scientific research, there's no evidence to suggest that Botox causes cancer. The confusion often arises from the word "toxin" being part of its name.

Can Botox cause headaches ?

While Botox is actually approved for the treatment of chronic migraines, there's a bit of an irony because, in some cases, it can lead to headaches post-treatment. These headaches are usually short-lived and tend to dissipate a few days after the injections. This side effects in majority of cases occurs for first-timers.

Can Botox goes wrong ?

While Botox is widely regarded as safe, complications can arise, just as with any medical treatment. Some common concerns include: - Unnatural facial expressions: Too much Botox or improper placement can lead to a "frozen" look. - Bruising and swelling: Generally minor but can be noticeable. - Drooping eyelids or eyebrows: Occurs when the Botox moves to areas (inappropiatelly executed injection) it wasn't intended for.

Are Botox top-ups free of charge ? 

This is where things get a bit industry-specific, and the answer isn't always straightforward. Generally, whether or not you get free top-ups depends on the clinic and the policy they have in place. Some places like odNOVA offer complimentary top-ups within a certain timeframe after your initial treatment, usually around 2 weeks, to ensure client satisfaction and to adjust the results if necessary. However, this is not a universal practice!

Are Botox injections safe ?

In short, when administered by a licensed and experienced professional, Botox injections are considered safe. The key here is ensuring you're in the hands of a reputable practitioner who understands facial anatomy and has a track record of successful treatments.

Are Botox injections painful ?

This is probably the most common question, and the answer can vary from person to person. Most people describe the sensation as a quick pinch or slight pressure when the injection is administered. It's relatively brief and manageable.
